AFTER COMPLETING THE ONLINE ENROLLMENT,

BELOW LISTS THE REQUIRED ITEMS NEEDED FOR THE ENROLLMENT APPOINTMENT:
1. Student’s Birth Certificate or Passport
2. Student’s Social Security Card
3. Immunization Records
4. Transcript of Grades from previous school
5. Any applicable state test scores (i.e. ISTEP scores from previous Indiana school)
6. Individual Education Plan (IEP) if applicable
7. Court Issued Custodial Documents (if applicable)
8. Proof of Residency - 2 verification documents required
(one from EACH of the following categories)
Housing Document
a) Rental Agreement
b) Purchase Agreement
c) Closing Document
d) Deed
e) Current Homeowners Insurance Policy
f) Current Parcel Ownership Report (available at the Hamilton Co. Government Web Site.)
AND
Utility Statement
a) Electric
b) Gas
All documentation is subject to verification.
NOTE: You must be a Carmel resident in order to enroll your student. If you have extenuating circumstances relating to your residency, please notify us prior to your appointment.
