Parent Meeting Notes

Requirements for participation

Students must be enrolled at Clay Middle School, complete a doctor's physical exam (dated after April 1st, 2014), complete a parental consent form, complete a concussion acknowledgement and signature form, maintain a 2.0 GPA, and contact Coach Carter to express an interest in participating. Physical, consent, and concussion forms are linked below.


Both the physical form and parent consent form can be found on the main athletics page on our Clay website. Please note that if a student needs to have his/her inhaler on hand during practice and meets, the asthma form needs to be completed by a doctor and submitted to the Nurse's office.
