Player clothing and spirit wear will be ordered the week of December 4, 2017-after the team has been selected.  Sample sizes will be available.  A link to the site will be added soon.

Important Information

 We can not accept in victory what we would not accept in defeat. 

     All participants trying out for basketball must dress appropriately. 
     Participants must  wear athletic shoes.
     Hair should be out of your face, and no jewelry should be worn

Tryout Packet
      In order to try out for basketball you MUST have the following 3 forms completed/turned in no
      later than November 28, 2017. 
      Athletic Physical (must be performed and dated after April 1, 2017) - turn into main office
      Online Consent Form & Concussion Form
      The online forms take 24 hours to process, so be sure to complete these as soon as

      We are excited to start a new season at Creekside Middle School and look forward to seeing

A committment to our team indicates that all non-school conflicts with our practice and game schedule will be resolved in favor of our school team. This would not, however, include religious obligations, family weddings or funerals. In other words, when you take a spot on this team, our schedule comes first. You are choosing for yourself, teammates, coaches, and Creekside to do just that.

You may expect all rules to be communicated, to be treated fairly and honestly, to work very hard-both mentally and physically, to learn a lot, and to have FUN.
