Expense Reimbursements

If you or your committee incurs PTO expenses, you must complete the following PTO Reimbursement Form and place in folder marked PTO Treasurer-Reimbursements in PTO basket in Work Room. 


As a nonprofit organization, the PTO is exempt from Indiana Sales Tax.   When making purchases for PTO purposes, advise the merchant you are “tax exempt” at the beginning of the transaction. At most retailers, such as Meijer, Target and Office Depot, the cashier can complete the transaction.  In some cases, you may need to go to Customer Service.  Be sure to take a copy of the Sales Tax Exemption Certificate with you as the retailer needs to view the document.  Use the link below to obtain the Certificate. All financial questions should be forwarded to the PTO Treasurer.
