School Immunization Requirements by the Indiana State Department of Health

Indiana law states that students in all grades are required to meet the minimum immunization requirements. The immunization record must include the student’s name, date of birth, and the vaccine given and date (month/day/year) of each immunization. Below lists the Indiana school requirements of vaccines with number of doses students must have before attending class.

Proof of immunizations must be submitted to the nurse at your child's school by August 1.

Notes

 All students in grades K-12 are required to have two (2) appropriately administered doses of varicella vaccine or evidence of immunity. For children entering preschool through 8th grade, a signed statement by a health care provider, documenting a diagnosis of varicella or verifying the history of disease, including date (MM/YY) is required. Parental report of disease history is acceptable for grades 9-12.

Kindergarten Immunization Requirements

  • 3 Hepatitis B
  • 5 DTaP (Diphtheria, Tetanus, and Pertussis)
  • 4 Polio
  • 2MMR (Measles, Mumps, and Rubella)
  • 2 Varicella (Chicken Pox)
  • 2 Hepatitis A

 

Reminder for Incoming Sixth Graders

All incoming sixth graders must have the following immunizations:

  • Meningococcal (MCV4)
  • Second Varicella
  • Tdap

 

MCV4 Requirement for Seniors

Seniors will be required to receive their second Meningococcal (MCV4) before August 1.

Individuals who receive dose 1 on or after their 16th birthday only need 1 dose of MCV4.
